CMS Releases Updated PBJ Manual To Allow For On & Off Site Medical Director Responsibilities To Count

Last week, the Centers for Medicare & Medicaid Services (CMS) released survey and certification letter 16-13 which reminds long-term care facilities that the mandatory submission period for electronic reporting on staffing begins on July 1, 2016. The Society for Post-Acute and Long-Term Care Medicine met with CMS and argued that the “on site” requirement for reporting Medicare Director hours could lead to inaccurate reporting.

CMS has amended this requirement in its recent changes to the Payroll-Based Journal (PBJ) – Implementation of required electronic submission of Staffing Data for Long Term Care (LTC) Facilities manual, which includes the removal of “on site” when describing the way Medical Directors hours are to be reported.

CMS Requires Payroll-Based Journal, Electronic Staffing Data Submission for Skilled Nursing Facilities, Effective July 1, 2016

Effective July 1, 2016, skilled nursing facilities (SNFs) will be required to submit staffing, time and attendance information electronically to the Centers for Medicaid and Medicare Services (CMS) via their Payroll-Based Journal (PBJ) system.

These requirements originate from section 6106 of the Patient Protection and Affordable Care Act (PPACA), and the final rule requires facilities to submit direct care staffing information, in a uniform and auditable format, at least quarterly. Early implementation is allowed; the CMS has been accepting electronic PBJ submissions since October 2015.

CMS announced on December 4 that the Five Star Quality Rating system will resume all updates with the January 2021 refresh. CMS UPDATE: Q2 2020 optional PBJ data due November 14

CMS has announced that Q2 2020 data (January 1 – March 31) is due no later than November 14. The alert was posted on the CMS PBJ portal and reads:

New Q2 Deadline: CMS has established November 14, 2020 as the new deadline for the Q2 2020 data.

CMS announces end of emergency blanket data waiver for nursing homes

Federal officials said Friday that they are ending an emergency blanket waiver intended to reduce administrative burden on nursing homes amid the continuing COVID-19 pandemic.

The Centers for Medicare & Medicare Services (CMS) said that effective August 14, nursing homes nationwide will again be required to submit staffing data through the Payroll-Based Journal (PBJ) system, which allows the agency to collect “staffing information which impacts the quality of care residents receive,” CMS said in a press release.

CMS Releases Nursing Home Staffing, Census Data to Help States Make PPE and Testing Decisions

The Centers for Medicare & Medicaid Services (CMS) on Friday released a public database of staffing and resident counts for nearly 15,000 nursing homes, with the goal of potentially helping states make more informed decisions about the distribution of personal protective equipment (PPE) amid the COVID-19 pandemic.

Using fourth-quarter 2019 data from the Payroll-Based Journal (PBJ) system, CMS compiled a spreadsheet that includes a variety of staffing information about each facility, including average daily nursing headcount, average daily overall staffing totals, the number of certified beds, and the average resident census.

Things to remember when submitting PBJ data

CMS will provide further information as soon as possible but are encouraging facilities to still report data as they are able. There will be no penalties or negative impacts to staffing ratings for not submitting by May 15.

CMS Focuses on Enforcement in Latest Statement on SNF Policies

n a blog post on the Trump administration’s strategies for ensuring safety and quality in skilled nursing facilities (SNFs), Seema Verma, administrator of the Centers for Medicare & Medicaid Services (CMS), focused on enhanced enforcement efforts to strengthen oversight of facilities.

On the issue of staffing, CMS is monitoring adherence to federal statute on the number of registered nurses that SNFs employ to help care for patients and residents.

PBJ update: Goodbye, self-reported census!

When CMS first implemented PBJ, facilities self-reported their census data for Five-Star staffing calculations. Since April 2018, CMS has automatically calculated the census instead. While facilities could still report their census, CMS did not use the data in its Five-Star staffing calculations.

A January 9 update to PBJ Data Submission Specifications now states CMS will no longer accept any census data in PBJ file submissions.
